When you have reviewed your exam and answered the questions in the evaluation interview preparation document, do the following:

1. If you have not already done so, download the Evaluation Interview Grid and enter your name and permanent code in the appropriate fields.
2. Come back here (“Evaluation Interview” page) and click Add submission.
3. Place your grid in the space provided and then click Save changes.
